A person stays accountable for the decisions that matter.
An approval engine for the points where automation has to stop and wait for a person to decide. Policy sets who may approve, how many of them, in what order, who may stand in for them and what happens when nobody answers. The requesting system keeps working while it waits, and all decisions are recorded with the name of the person who made them.
